The survey is anonymous. There is an optional question asking for those willing to provide their email address so that we may contact them directly for follow-up questions, but this is not required to complete the rest of the survey. Entering the prize draw does require entering your email address. Sensitive questions always include a “prefer not to say” option. Personally identifying information is encrypted at rest.
